What Is ICSI? When IVF Alone Isn’t Enough
In standard IVF, thousands of prepared sperm are placed around each egg in a dish, and fertilisation happens on its own. ICSI — intracytoplasmic sperm injection — takes a different approach entirely: one selected sperm is injected directly into one egg, by hand, under a microscope. It’s the solution when sperm can’t complete the job alone, and it’s now the most commonly used fertilisation method in IVF labs worldwide.
How It Actually Works
After egg retrieval (identical to standard IVF), the embryologist strips each egg of surrounding cells and confirms maturity. Under 400x magnification, they select a single sperm — judged by shape and movement — immobilise it, draw it into a glass needle a fraction of a hair’s width, and inject it through the egg’s outer shell into the cytoplasm. Each egg takes about a minute. It’s a manual skill, performed hundreds of times by experienced embryologists, and the quality of those hands directly affects your results.
Who Needs ICSI?
- Low sperm count — when there aren’t enough sperm for conventional IVF to work reliably.
- Poor motility — sperm that can’t swim well enough to penetrate the egg on their own.
- Abnormal morphology — too few normally shaped sperm for natural fertilisation; ICSI lets the embryologist select the best available.
- Azoospermia with surgical retrieval — sperm from TESA or Micro-TESE comes in small numbers and often with limited motility. ICSI is the only route.
- Previous fertilisation failure — a conventional IVF cycle where few or no eggs fertilised is the classic reason to switch to ICSI.
- Frozen or limited sperm — banked samples, cancer-survivor samples, or donor samples in short supply.
- PGT cycles — genetic testing requires ICSI to prevent stray sperm DNA from contaminating the biopsy.
Who Doesn’t Need ICSI
Couples with normal semen parameters. Multiple large studies show no improvement in pregnancy rates with ICSI over conventional IVF when sperm is healthy. If a clinic recommends ICSI by default without a sperm-based reason, ask why — the answer reveals whether the recommendation is clinical or financial. ICSI adds ₹20,000–40,000 to the cycle cost; unnecessary ICSI adds cost without babies.
ICSI Success
Fertilisation rates in good labs: 70–80% of injected mature eggs. That’s the ICSI-specific number and it’s reliably strong. Pregnancy rates after transfer depend on the same factors as any IVF: her age, embryo quality, uterine receptivity. For couples where male factor is the primary problem and female age is young, ICSI outcomes are often excellent — because the technique removes the only barrier.
