Vitamin D and Fertility — Does Supplementation Help?

Vitamin D deficiency is one of the most common nutritional gaps in India — affecting an estimated 70–90% of the population, with women of reproductive age particularly affected. At the same time, a growing body of evidence links vitamin D status to fertility outcomes. The connection isn’t as dramatic as supplement marketers claim, but it’s real enough to act on — especially since testing is cheap and supplementation is safe.

What the Evidence Actually Shows

Multiple studies — including meta-analyses pooling thousands of IVF cycles — find that women with sufficient vitamin D levels (above 30 ng/ml) have modestly but consistently higher clinical pregnancy rates and live birth rates compared to deficient women. The effect size isn’t enormous (roughly 30–35% higher odds of live birth in some analyses), but given how cheap and safe supplementation is, the risk-benefit calculation is lopsided in favour of action.

The proposed mechanisms: vitamin D receptors exist in the ovaries, uterus, and placenta, suggesting direct roles in follicle development, endometrial receptivity, and implantation. Deficiency also correlates with insulin resistance — relevant for PCOS patients, where vitamin D supplementation has shown improvements in ovulation rates alongside other treatments.

For Male Fertility Too

Vitamin D receptors are present in testicular tissue, and deficiency is associated with lower sperm count and motility in some studies. The evidence is less robust than for women, but given the absence of downside, men in a fertility-planning couple should test and supplement if low. How to Act on This

Step 1: Test. A 25-hydroxy vitamin D blood test costs ₹500–1,000 in Ahmedabad. Ask for it alongside your fertility workup — most clinics don’t include it automatically, but should.

Step 2: Interpret. Below 20 ng/ml is deficient; 20–30 is insufficient; above 30 is sufficient. For fertility purposes, many specialists target 40–60 ng/ml.

Step 3: Supplement if needed. Typical dosing: 1,000–2,000 IU daily for maintenance; 60,000 IU weekly for 6–8 weeks to correct deficiency, then maintenance. Always with your doctor’s guidance — vitamin D is fat-soluble, so unlike vitamin C you can take too much (though toxicity is rare at sensible doses).

Step 4: Retest after 8–12 weeks to confirm levels have normalised.

What Vitamin D Won’t Do

It won’t fix blocked tubes, reverse severe male factor, or compensate for age-related egg decline. It’s an optimisation — a genuine one, with real evidence behind it — not a treatment. The couples who benefit most are those who correct a deficiency alongside appropriate medical treatment, not instead of it.
