Unexplained Infertility — When All Tests Are Normal

Everything tests normal — her tubes are open, ovulation is regular, AMH is fine, his semen analysis is good — and still no pregnancy. Unexplained infertility is the diagnosis for roughly 25–30% of infertile couples, and it’s one of the most frustrating labels in medicine: you came for answers and got a shrug. Here’s what it actually means and, more importantly, what to do about it.

What “Unexplained” Really Means

It doesn’t mean “nothing is wrong.” It means the standard tests — semen analysis, ovulation confirmation, tubal patency, basic hormones — haven’t found the problem. The limitation is in the testing, not in your body. Possible hidden causes that standard workups don’t catch:

The Treatment Pathway — Evidence-Based

Step 1: Timed Intercourse with Mild Stimulation (3 cycles)

Ovulation-inducing medication (letrozole or clomiphene) paired with timed intercourse — the stimulation produces 1–2 extra follicles and ensures precise timing. Success: roughly 8–12% per cycle, cumulative 25–35% over three cycles. Low cost, low intervention.

Step 2: IUI with Stimulation (3–4 cycles)

IUI adds sperm washing and bypasses the cervix. Combined with stimulation, per-cycle rates reach 12–18%. The standard protocol for unexplained infertility: 3–4 IUI cycles, with a firm pre-agreed exit to IVF. Total cost for 3 cycles: ₹40,000–80,000.

Step 3: IVF (if steps 1–2 fail)

Here’s where unexplained infertility often stops being unexplained. The IVF lab reveals what standard tests couldn’t: poor fertilisation (egg or sperm surface issue), arrested embryo development (quality problem), or excellent embryos that don’t implant (endometrial factor). Each failure generates data; each cycle can be adjusted. IVF for unexplained infertility has good success rates — often better than for identified causes, because both partners’ fundamental biology is intact.

The Timeline Mistake

The biggest error in unexplained infertility is drifting. Three months of “let’s just try naturally” becomes six, becomes twelve, becomes “we should have started IUI a year ago.” The treatment ladder exists for a reason: each step has a defined duration and a pre-agreed escalation. Total time from step 1 to IVF entry should be 9–12 months maximum — not two years of repeating the same step hoping for a different outcome.

The Encouraging Truth

Unexplained infertility has a better prognosis than most diagnosed causes. Treatment works — most couples with unexplained infertility conceive with IUI or IVF within 2–3 years of starting treatment. And the “unexplained” part often resolves itself in the IVF lab, where the reason becomes visible for the first time. What feels like the worst diagnosis (no answer) is often the best prognosis (nothing fundamentally broken).
