Fibroids and Fertility — When They Matter and When They Don’t
Uterine fibroids — benign muscle growths in or around the uterus — are extremely common, affecting up to 70% of women by age 50. Most cause no fertility problems at all. The question isn’t whether you have fibroids; it’s where they are. Location, not size, determines whether they interfere with conception and whether treatment is needed before fertility attempts.
The Three Types by Location
Submucosal (Inside the Cavity) — These Matter
Fibroids that grow into or distort the uterine cavity — the space where embryos implant — have the clearest evidence of impairing fertility. They physically interfere with implantation, alter blood flow to the lining, and may cause early miscarriage. Even small submucosal fibroids (1–2 cm) can reduce IVF implantation rates by 30–50%. Recommendation: remove before fertility treatment. Hysteroscopic myomectomy — a camera through the cervix, no abdominal incision — removes cavity-distorting fibroids as a day procedure. Recovery: a few days. Fertility attempts can resume within 1–2 months.
Intramural (Within the Muscle Wall) — Depends on Size
The majority of fibroids. Small intramural fibroids (under 4–5 cm) that don’t distort the cavity have minimal evidence of affecting fertility — most specialists leave them alone. Larger ones (above 5 cm) that push into the cavity or distort uterine shape may warrant removal, but the evidence is less clear-cut, and the decision is individualised. Surgery here means abdominal myomectomy (laparoscopic or open), which requires 2–3 months of recovery before conception and may necessitate caesarean delivery.
Subserosal (Outside the Uterus) — Usually Don’t Matter
Growing outward on the uterine surface, these rarely affect fertility unless they’re very large (compressing tubes or adjacent structures) or pedunculated (on a stalk, risking torsion). In most cases, they’re left alone for fertility purposes.
How Fibroids Are Detected
Transvaginal ultrasound: the first-line tool — identifies most fibroids and their approximate location. Saline infusion sonogram (SIS): saline fills the cavity, clearly outlining any submucosal component — the best non-surgical test for cavity distortion. MRI: the gold standard for mapping exact size, number, and position when surgery is being planned. Hysteroscopy: direct camera view inside the cavity — diagnostic and therapeutic in one step.
Fibroids and IVF — the Practical Impact
If your fibroids are intramural-only and the cavity is clean on SIS: proceed with IVF without surgery. Unnecessary myomectomy delays treatment, creates scarring, and reduces uterine capacity — a cure worse than the disease when the fibroids weren’t the problem.
If there’s a submucosal component: remove it first. The improvement in implantation rates is well-documented and the procedure is minor.
If a large intramural fibroid is present: discuss with your specialist — some will recommend removal (if it’s displacing the cavity or compressing blood supply); others will attempt IVF first and consider surgery only if implantation fails repeatedly.
Fibroids During Pregnancy
Most fibroids don’t cause pregnancy complications. Some grow under hormonal influence (usually in the first trimester) and can occasionally cause pain (degeneration), malpresentation, or placental issues. Your obstetrician monitors them — the presence of fibroids doesn’t automatically make a pregnancy high-risk.
