Why Urgency Matters

Chemotherapy drugs — especially alkylating agents — and pelvic radiation directly damage eggs and sperm-producing cells. The damage can be temporary (production resumes after treatment) or permanent (premature ovarian failure, azoospermia). The problem: you can’t predict which outcome you’ll get before treatment starts. Banking eggs, sperm, or embryos before the first dose preserves the option regardless of what treatment does to your fertility afterward.

For Women

Egg Freezing (10–14 days)

A stimulation cycle can begin at any point in the menstrual cycle using random-start protocols — no need to wait for a period. Injections for 10–12 days, egg retrieval, vitrification. Embryo Freezing (same timeline, with a partner)

If you have a committed partner and want to maximise the future chance per stored unit, fertilising the eggs and freezing embryos gives a slightly stronger statistical foundation. Consideration: embryos require both partners’ consent for future use. If relationship certainty is anything less than absolute, freezing unfertilised eggs preserves your sole autonomy.

Other Options

Ovarian tissue freezing: for pre-pubertal girls or women who cannot delay treatment even two weeks. Ovarian tissue is surgically removed, frozen, and later reimplanted — an established technique with documented live births, available at specialised centres.

GnRH agonist co-treatment: given alongside chemo to suppress the ovaries, potentially reducing damage. Evidence is promising but not a substitute for egg/embryo banking — use it in addition, not instead.

For Men

Sperm Banking (1 day)

One clinic visit. One sample, split into multiple vials, frozen in liquid nitrogen. Total cost: ₹5,000–15,000 including first-year storage. If time allows, two or three samples banked a day or two apart multiply the reserve. Even men with already-reduced counts (from the disease itself) should bank — ICSI needs only a handful of viable sperm per egg.

The Conversation With Your Oncologist

Many oncologists focus — understandably — on the cancer, and fertility preservation can fall through the cracks. Bring it up explicitly: “Can we discuss fertility preservation before treatment starts?” If your oncologist says the treatment timeline doesn’t allow it, ask specifically how many days are available — two weeks is usually enough for egg freezing, and one day is enough for sperm banking.
