Natural and Mini IVF — Less Medication, Lower Cost, Different Trade-offs
Not every IVF cycle needs maximum stimulation. Natural IVF uses your body’s own single egg — no injections at all. Mini (mild) IVF uses low-dose medication to produce 3–5 eggs instead of the standard 8–15. Both cost less, involve fewer drugs, and carry lower side-effect risk. The trade-off is straightforward: fewer eggs means fewer embryos, which means fewer chances per cycle. Here’s when that trade-off works in your favour.
Natural IVF — Zero Stimulation
How: monitoring your natural cycle with ultrasound to track the single dominant follicle. Trigger injection when mature, retrieval of one egg (sometimes two), ICSI fertilisation, and transfer if a blastocyst develops.
Cost: ₹50,000–₹80,000 — dramatically lower than conventional IVF because there’s no medication cost.
Per-cycle success: 7–15% — lower than conventional IVF because you’re working with one egg and its single chance of being chromosomally normal, fertilising, developing, and implanting.
Best for: women who respond poorly to stimulation anyway (very low AMH where high-dose medications yield only 1–2 eggs at much higher cost); women who want to avoid hormones for personal or medical reasons; younger women willing to try multiple low-cost cycles rather than one expensive one.
Mini (Mild) IVF — Low-Dose Stimulation
How: oral medication (letrozole or clomiphene) with or without very low-dose injectable gonadotropins for 5–9 days. Aims for 3–5 mature follicles. Retrieval, fertilisation, and transfer proceed as standard.
Cost: ₹70,000–₹1,20,000 — medication costs drop significantly, often to ₹5,000–15,000 instead of ₹40,000–80,000.
Per-cycle success: 15–30% — higher than natural IVF because more eggs give more chances, but lower than conventional because the embryo pool is smaller.
Best for: good responders who produce quality eggs easily (standard protocol’s additional eggs don’t always translate to additional good embryos); women concerned about OHSS risk (less stimulation = less risk); couples pursuing multiple affordable cycles rather than one high-investment round.
When Standard IVF Is Still the Better Choice
- Women 37+: at this age, the proportion of chromosomally normal eggs is dropping. You need more eggs to find the normal ones — standard stimulation maximises the yield per retrieval, reducing the total number of cycles (and months) needed.
- PGT-A planned: genetic testing needs multiple embryos to be useful — testing one embryo from a natural cycle rarely adds value.
- Male factor requiring ICSI: with ICSI costs fixed per cycle, spreading them across more eggs (standard IVF) is more cost-effective.
- Time pressure: if achieving pregnancy quickly matters (age, career, personal timeline), more eggs per cycle means faster progress toward a viable embryo.
The Cumulative Cost Argument
Mini IVF advocates argue that three mini cycles at ₹1 lakh each (₹3 lakh total) may produce the same cumulative success as one conventional cycle at ₹2.5 lakh — at similar cost but spread over time, with fewer side effects per cycle and less emotional weight per attempt. The counter: three cycles means three retrievals, three rounds of monitoring, and 3–4 months instead of one. For younger women with time, the maths can work. For older women racing the biological clock, compressing into one high-yield cycle is usually wiser.
